Outreach Patient Navigator - Full-time
Telecommute 32-40 hours per week
ICMS is looking for an inventive, creative and innovative employee who looks for unique ways to improve the overall health of our patients.
This position will work with patients, healthcare providers, and community resources to remove challenges of obtaining cancer screenings and referral coordination. The Navigator will work with patients to identify resources that would remove any barriers to care, such as identifying financial assistance programs, transportation, and social support.
-Identifies patients with gaps in care and those due for screenings.
-Reaches out to community members to promote healthcare and schedule medical appointments.
-Identifies barriers that may be preventing patients from obtaining tests (inadequate insurance, transportation, fear, etc.).
-Provides enrollment/eligibility assistance.
-Collaborates with care managers or other social service agencies to reduce any barriers.
-Helps patients explore alternative test options and care management strategies.
Qualifications
• Experience working in a medical setting preferred.
• Warm and effective communicator, with the ability to interact effectively and in a supportive manner with persons of all backgrounds.
• Good telephone skills.
• Ability to respect and to maintain privacy and dignity of patients and ensure patient confidentiality at all times.
• Computer skills - Microsoft Word, Excel, and data entry.
• Ability to complete work accurately and in a timely manner.
• Work history of dependability and good attendance.
• Self-discipline to manage own time.
• Remote work environment that allows focus and is free of interruptions.
