We are your Medical Home!

The dedicated staff of Islands Community Medical Services work hard everyday to be Your Medical Home.  We do so by utilizing a model of care that is actually referred to as the Patient-Centered Medical Home model. 

At its core, this model is a way of saying that you, the patient, are the most important person in the health care system. You are at the center of your health care. 

To support your efforts in establishing and reaching your goals, we have:
  • Built a strong team that includes medical, behavioral health, oral health and physical therapy experts
  • Established relationships with specialists, hospitals and community organizations that can help us in supporting your needs outside of the walls of 15 Medical Center Loop.  We call this your medical neighborhood.
  • Put into place a comprehensive system to track your care and your progress toward your goals, and to assess our efforts in helping you achieve your healthiest lifestyle 

There are five hallmarks of the Patient-Centered Medical Home model and we have included them so you know what ICMS is doing for you!

  1. Comprehensive Care.  We are accountable for meeting the large majority of your physical and mental health care needs, including prevention and wellness, acute care, and management of chronic diseases. We are only able to do this because we are dedicated to working together as a team – YOUR care team.

  2. Patient-Centered.  Partnering with you, and your loved ones - if you so choose –provides us with a better understanding and respect for your unique needs, values, and preferences. We support you in learning to manage and organize your own care at a level you are comfortable with. We recognize that you and your loved ones are core members of your care team, and we work to ensure that everyone is a fully informed partner in establishing care plans.

  3. Coordinated Care.  We coordinate care across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports.  This work means partnering with you to support you with scheduling visits outside of the medical center, and in making sure that valuable information about your health finds its way back to your care team.

  4. Accessible Services.  We work to deliver services with shorter waiting times for urgent needs, provide around-the-clock telephone or electronic access to a member of the care team, as well as alternative methods of communication such as telephone care and even an online patient portal.

  5. Quality and Safety.  We are committed to quality - and quality improvement - by using evidence-based medicine and clinical decision-support tools to guide decision making among you and your loved ones.  We also engage in performance measurement efforts to gauge your experiences and satisfaction and our ability to provide high quality care.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
